Community Health Education & Healthy Living
At the Foothills Health District, we believe that data drives action—and that informed communities are healthier communities. Through our Community Health Education efforts, we assess the health needs of Rutherford and McDowell Counties, set priorities, and work alongside partners to improve the well-being of everyone we serve.
This section includes our key planning and reporting documents:
- Community Health Assessment (CHA): Conducted every three years, the CHA identifies the top health issues affecting our residents by analyzing local data and community input.
- Community Health Improvement Plan (CHIP): Based on the CHA, the CHIP outlines measurable goals and strategies to address our counties’ top health priorities.
- State of the County Health (SOTCH): Reports: Published annually between CHAs, the SOTCH tracks progress, highlights new initiatives, and updates the community on emerging health trends.
These reports help guide local public health efforts, foster collaboration, and support accountability as we work to create healthier, more equitable communities.
