Please print out and complete the Complaint Form (FH 4001.010) 

Email the completed form to the county office location where the property is located:

Include with the form any photo evidence that you may be able to provide for documentation.

Important Information:

  • Complaints with false names, anonymous, or blank names will not be given high priority
  • A complaint to the Environmental Health Department is a public record and will be treated as such
  • The Environmental Health Department is not responsible for complaints involving animals, trash or refuse
  • A complaint to the Environmental Health Department does not grant right-of-entry onto any piece of property
